Submit a Request

Submit a Request is a secure tool for users to send questions or requests to Health Endeavors. We prefer users to send requests to us via the Submit a Request tool rather than email to ensure your request is routed to the proper team in Health Endeavors and also ensure your request is processed in a timely manner. The Submit a request tool is available to all users. 

To Access Submit a Request, click on the Admin tab in the upper right hand corner of the screen and scroll down and click on Submit a Request

Once in Submit a Request, you will see three tabs, Current, Closed and Submit a Request.

 

Current shows all your open Submit a Requests with Health Endeavors, Closed shows all previous requests that have been resolved and Submit a Request is for creating a new request.

To create a new request, click on the Submit a Request tab. All fields are required. First enter a subject for your request. In the Description of the Request, enter as much detail as possible. This will ensure we can quickly respond with an answer to your request. You may also attach up to three files to your Submit a Request. Once you have completed your request click the Send Request button. Health Endeavors will receive notification of your request and respond as quickly as possible.

After you click Send Request you should receive an email with notification that your request was sent. Once Health Endeavors responds to your request, you will also receive an email that there was a response to your request. You will need to login to Health Endeavors to review our response and then have the ability to either send additional responses on this request, or close the ticket if your request has been resolved.

 

Please note that Submit a Request is a secure tool, so if you may send patient information securely unlike an email.

Health Endeavors may send a request on your behalf with requests from meetings. You will find these requests located in the current tab for review.

When viewing an open or closed ticket, click on the 'View Details' button for the ticket you would like to view.

Once you are in the ticket you may notice a second 'View Details' button. Click on this button to show ticket details and attachments that may be a part of the ticket but hidden from the initial view.

After you have received a response from Health Endeavors you may need to send additional information or have additional questions. Enter your updates in the 'Reply' box and click the Submit button to send back to Health Endeavors. Or, if your request is completed, click the Close button to close the ticket.

If a ticket is closed and you find you still have questions, please open a new ticket and reference the original ticket in your request. This will help us gather details to answer your request quickly.
